$395 applies to one child age 3 and up. For children under age 3 please call the office for pricing.

Preventive: This plan offers 2 preventive visits a year. Each visit includes exam, routine x-rays, cleanings and fluoride treatment.

Emergencies: One emergency visit per year, includes one x-ray.

Pediatric Dental Services: 15% discount on all dental services.

Orthodontic Services: Includes one free orthodontic exam and 15% off orthodontic services

Membership period starts immediately and is active for 1 year. Benefits expire after a year and must be used during the membership period. You can request a refund within 30 days if no benefits have been used. After 30 days no refund will be issued. Membership plan is for one child and is non-transferable. Other exclusions may apply.
